WE NEED YOUR HELP
We are trying to determine the number of food products that claim "No MSG" or state "No MSG added" or "No added MSG" on their labels. Further, we are trying to determine how many people have purchased such products believing the label claims regarding MSG, and how many people have had MSG type reactions from such products.
Many such products are mislabeled in that they contain processed free glutamic acid (MSG).
Help us by letting us know of any such products you have encountered and if you ever experienced an MSG type adverse reaction from such products.

1. Your name
2. The city, state, and
country in which you live
3. Your phone number so
we may contact you, if necessary
4. The name of the product,
including the name of the manufacturer (for example, Umper's Beef Gravy
Mix)
5. Whether or not you tried
the product
6. The approximate date
you tried the product, if you tried it
7. Whether or not you experienced
an MSG type reaction from the product
8. The reaction you experienced
from the product, if any
9. The label claim: "No
MSG," "No added MSG," or "No MSG added"
10. If available, the ingredients listed
on the product label
